In Block mode (the default), yes, for forms it classifies as PHI. Use Flag mode to see the results first. Personal-information forms are reported but not blocked unless you choose to.

Only if entry sampling is on, and only to look for patterns. It keeps the pattern name and a count, never the answer, and never stores, logs, or sends answers anywhere.
