Not unless you ask it to. By default the plugin only hides review functionality, and deactivating it brings everything back exactly as it was. Since version 1.1.0 there is a separate "Clean Up Existing Reviews" tool on the settings page. It never runs on its own: you have to press the button, and you can choose between moving reviews to the trash (restorable from the Comments screen) or deleting them permanently. Take a backup before deleting.

Selecting a parent category automatically covers its child categories. Everything you don't list stays review free, including under the "Disable All Review Features" master switch. While exceptions are active the Products → Reviews admin screen stays available, so you can still moderate the reviews that are coming in.
