There's one global form, configured from Settings > Mozedia Forms.
This covers the common case (e.g. a General Contact form and an "Advertise With Us" form that emails a different address) without a full multi-form system.
Nowhere. Submissions are emailed to the configured recipient via wp_mail() and are not stored in the database. There is no submissions log to review in wp-admin.
Many local/development environments (including localhost) don't have a working mail transport configured, so wp_mail() fails even though the plugin is working correctly. Use the Send Test Email button on the Mail tab to check - it will show you the underlying error. On a live website, this usually resolves itself; if not, configure custom SMTP on the Mail tab.
Honeypot protection (an invisible field bots fill in, plus a minimum-time check) is on by default and needs no configuration. You can also enable per-IP rate limiting and message-content rules (max length, link limits) from the Protection tab.
No. With AJAX off, the form submits as a normal page reload, using a Post/Redirect/Get pattern - so refreshing the result page never resends the form. Success/error messages and field-level errors still show, right where the form was.
Yes. The form and its fields use stable, predictable class names (like mzforms-form-wrapper, mzforms-form, and mzforms-field) that you can target with your own CSS in your theme, so you're not stuck with the default look.
Yes, with per-site settings. Network-activating the plugin seeds default settings on every site, but each site's admin configures its own Mozedia Forms settings independently under Settings - there's no network-wide settings page to manage every site at once.
Not unless you turn on "Delete Data on Uninstall" on the Advanced tab first. By default, your settings survive a plugin deletion and reinstall.
