Yes. Go to Survey Forms → Surveys and add one there instead of building it straight onto a page. Each entry holds a single survey and gets its own shortcode, shown in the ShortCode column of the list — click it to copy. Paste [survey-form-block id="123"] into any page, post, widget, or into a theme template with do_shortcode(), and the survey appears there. Every copy writes to the same set of responses, and editing the survey once updates all of them.

No. Dropping the Survey Form Block straight onto a page works exactly as it always has, and surveys built that way are untouched by this. The Surveys screen is there for when you want one survey in several places.

Nothing - they are kept. Deleting the plugin leaves your surveys, responses and tables in place unless you opt in first, under Survey Forms → Demo & Help → Settings. That switch is off by default and asks you to type DELETE before it turns on.
