The plugin simply works upon activation. You may set the default featured image for each public post type under 'Settings' -> 'Media' sub-menu of the WordPress admin area.

Of course! For any post you want to disable smart featured image, just set a no_sfi custom field to any value other than 0. To restore smart featured image functionality, simply delete the no_sfi custom field, or set to 0, for that post.
